I Requirements for first time applicants	Seventeen (17) years old and below Birth Certificate - NSO Marriage Contract - NSO (If legitimate child) Affidavit of Illegitimacy (If parents are not married) Original School ID or minor (8 to 17 years old) Original passport of either parent Notarized affivadit of support and consent and original passport or valid identification card of mother is required, if father is one who appear to the DFA. If mother is abroad, affivadit of support and consent and photocopy of her passport (front and last page) must be autheticated by the nearest Philippine Embassy or General Consulate. if both parents are abroad, person applying in behalf of the minor must submit a Special Power of Attorney and affidavit of support and consent authenticated by the nearest Philippine Embassy or Generate Consulate plus the original passport of person traveling with the minor plus DSWD clearance. 4 pieces Passport size picture (Royal blue background; Photo should show applicant in decent attire. Wear colored dress with collar; plain white dress is not acceptable). NOTE:  Only the mother; father or person applying in behalf of the minor is required to appear to the DFA if minor is seven (7) years old and below. Eighteen (18) years old up to Sixty Four (64) year old Birth Certificate - NSO Marriage Contract - NSO (If married woman) NBI (for travel abroad) Original Identification Card (at least 2 ID) Example: School ID Company ID SSS ID PRC ID Voter's ID Driver's License Old NBI Old Postal ID Senior Citizen ID 4 pieces Passport size picture (Royal blue background; photo should show applicant in decent attire. Wear colored dress with collar; plain white dress is not acceptable). NOTE:  If Birth Certificate has no record, submit the Negative Record of Birth (NSO and LCR); Joint affivadit of birth; two old docs showing Date and Place of birth (for Senior Citizens only). Supporting Documents for First Timer Applicant Submit 2 old documents showing date and Place of birth if the Birth Certificate is late registered. Example of 2 old documents Baptismal Certificate Form 137 / Transcript of Record Voter Registration Record (COMELEC Main-Intramuros)
II. Requirements for renewal of passport	Original Passport of Applicant Original Passport of either Parents (for 17 years old and below) Photocopy of Birth Certificate (for 17 years old and below) Photocopy of Marriage Contract of Parents (for 17 years old and below) 4 pieces Passport size picture (royal blue background; photo should show applicant in decent attire. Wear colored dress with collar; plain white dress is not acceptable). NOTE:  Personal Appearance is required except for 7 years old and below and 65 years old and above. Personal Appearance of either parent is required for minor 7 years old and below
III. Requirements for replace of lost passport	Birth Certificate - NSO Marriage Contract - NSO (If married woman) Affivadit of loss Police Report (For Loss Valid) NBI (for travel abroad) Original Identification Card (at least 2 ID) 4 pieces Passport size picture (Royal blue background; Photo should show applicant in decent attire. Wear colored dress with collar; plain white dress is not acceptable).
